6月診療日のお知らせ。6/26（水曜日）と6/27（木曜日）全日休診。

2024年6月診療日のお知らせ。
2024/6/26（水曜日）から日独皮膚科学会が日本で開催される予定で、院長も参加します。
そのため、
6/26（水曜日）と6/27（木曜日）全日休診となります。
6/28（金曜日）から通常営業になります。
ご迷惑をお掛けしますが、ご理解のほどよろしくお願い致します。

2024年3月1日からマイナンバーカードをマイナ保険証として利用可能になります

2023年4月の保険医療機関のオンライン資格確認の原則義務化に伴い、2024年3月1日から当院でもマイナンバーカードをマイナ保険証として利用可能になります。
マイナ保険証の提出・同意の有無で初診時の加算点数が変更になります。
マイナ保険証のメリットもございますので、今後ともよろしくお願い致します。

☑️マイナンバーカード持参あり・診療情報取得同意なし→初診４点加算

※医療情報・システム基盤整備体制充実加算
初診時:【同意なし】４点 【同意あり】２点
再診時:【同意なし】０点 【同意あり】０点
1点は10円で、総診療点数により負担金額が変わります。

３月よりオンライン診療

2024年３月よりオンライン診療を開始します。詳しくはホームページまたはインスタグラムに公開致します。ホームページへの公開が遅れそうですので、その際はインスタグラムで当院のオンライン診療のシステムや流れを確認して下さい。北海道から沖縄まで全国どこからでもオンライン診療を受けられます。ただし、対面診療（病院での通院）が必要な場合もありますので、ご心配な方は一度クリニックにご相談下さい。当院では「デジスマ診療」アプリを用いて、オンライン診療を行いますので、アプリをダウンロード・インストールできるスマートフォンやiPadなどのタブレット端末をお使い下さい。ビデオカメラ機能の付いたコンピューターでも診療は可能ですが、病変部が見えるようにする必要があります。

２月から第２・第４土曜日の午後をオープン致します。

2024年２月から第２・第４土曜日の午後もクリニックをオープン致します。午後の受付は平日と同じ15：00〜18：30です。平日受診が難しかった方、土曜日午前中も学校で来れなかった方にも皮膚病を治して頂きたく、これからも地域医療に貢献したいと思います。なお、3/22（金曜日）は、ご迷惑をお掛け致しますが、午前お休みになります。　院長

健康保険証またはマイナンバーカード
また、以下をお持ちの方は健康保険証と一緒に提出してください。
紹介状、他の医療機関での検査結果、お薬手帳、各種医療証

初診、および月初めの受診時には､必ず健康保険証をお持ちください。
健康保険証に変更があった場合(住所、勤務先など）は速やかにお知らせください。
診察券はご来院の度に受付にご提出ください。

Please submit your insurance card if you have the below documents:
a referral form
results of medical examinations by other medical institution
a medication notebook
a variety of free/reduced medical care certificates

Please bring your health insurance card to your first consultation of each month.
Please let us know immediately if there are any changes such as address, your work, etc., in your personal information on your health insurance card.
Please submit your clinic ID card every time you come to the clinic.

お車でお越しの方へ

お車でお越しの場合は銀座松竹スクエア時間貸し地下駐車場や周辺のパーキングをご利用いただけます(駐車券のサービスはございません）。当クリニックとしての駐車場の確保はなく、満車の場合もございますので、ご注意ください。ただし皮膚症状や処方/治療内容により運転に支障が生じることがございますので、お車での来院はできるだけお控えください。

If you come here by car, you can use the pay-by-the-hour underground parking lot in the Ginza Shochiku Square or other carparks about here. (We do not have any carpark tickets service.) Please note that we do not have our allocated parking lots, also carparks may be full.
Please do not visit our clinic by car in case of difficulty in driving due to medication/treatment or skin conditions.

帯状疱疹ワクチン

帯状疱疹ワクチン（シングリックス）1回22,000円
・50歳以上の人：0.5mLを2回、通常2ヵ月の間隔をおいて筋肉内に接種します。
・帯状疱疹に罹患するリスクが高いと考えられる18歳以上の人（疾病や治療により免疫に異常がある人、免疫機能が低下した人または低下する可能性がある人）：0.5mLを2回、通常、1〜2ヵ月の間隔をおいて、筋肉内に接種します。

注）1回目の接種から2ヵ月を超えた場合であっても、6ヵ月後までに2回目の接種を受けてください。

A Shingles Vaccine
A Shingles Vaccine (SHINGRIX for I.M. Injection) 22,000 yen/shot
・person over 50 years old: injecting 0.5ml x 2, usually given intramuscularly at 1~2 months interval
・person over 18 years old at high risk of developing shingles (immunological abnormality due to illnesses or medical treatments, a person whose immune system is weakened or possibly weakened): injecting 0.5ml x 2, usually given intramuscularly at 1~2 months interval

【Attention】 Please take the second shot within 6 months if you failed to take it around 2 months after the first shot.

⑤ ドロップスクリーン（小児・乳幼児のアレルギー検査【41項目】もできます）

デュピクセント：アトピー性皮膚炎・結節性痒疹
ゾレア：慢性蕁麻疹
エピペン：アナフィラキシー
対応します。
DUPIXENT (Dupilumab) for atopic dermatitis and nodular prurigo, XOLAIR for chronic urticaria/hives, and EPIPEN for anaphylaxis are available.

紫外線治療：全身型ナローバンドUVB照射とエキシマライト（最新型のUSHIO: Therabeam UV308 mini LEDを導入しています)を取りそろえております。
Narrowband-UVB irradiation and excimer light by USHIO's latest Therabeam UV 308 mini LED are installed.

紫外線治療の対象疾患：アトピー性皮膚炎、尋常性乾癬、掌蹠膿疱症、尋常性白斑、円形脱毛症、類乾癬、菌状息肉症、悪性リンパ腫、慢性苔癬状粃糠疹

夜間・早朝等加算 / Out-of Hours Additional fees

平成22年4月から社会保険診療報酬等の法改正により、クリニックで下記の時間帯に受付された場合は、夜間・早朝等加算の取り扱いとなりますので、ご了承ください。

＊ 土 曜　12時以降

According to a revision of law on Social Insurance Health Fee, etc. in April 2010, we will charge additional fees if you visit here during the following hours:
* Weekdays After 18:00
* Saturdays After 12:00

Appointments

Our clinic does not require appointments.
We generally ask you to wait for your turn on the day.
Due to this, we have a limited number of appointments available.
Please visit the clinic during our reception hours (at the latest 30 minutes before closing time) in case the appointment booking system is full.
Please note that extra waiting hours might be required due to a medical examination without health insurance, medical treatment, etc.
Young children, pregnant women and disabled people will be prioritized during busy periods. Thank you for your cooperation and understanding.

Newly opened clinic

We newly opened the Total Skin Care Clinic Ginza on Thursday 1 June 2023 on the ground floor of the Ginza Shochiku Square.
